Varicose Veins

Varicose veins are swollen, twisted veins visible under the skin — usually on the legs. Endovenous laser ablation (EVLA) permanently closes the faulty vein in under an hour.

Overview

Varicose veins develop when the one-way valves in veins weaken, allowing blood to pool rather than flow back to the heart. This causes the veins to stretch, twist, and become visible as blue or purple bulges. Beyond cosmetic concerns, varicose veins cause pain, heaviness, itching, swelling, and can lead to skin changes and ulcers if left untreated.

EVLA is the gold standard treatment. A thin laser fibre is inserted into the diseased vein under ultrasound guidance. Laser energy heats and seals the vein shut. Blood naturally reroutes through healthy veins. The closed vein is gradually absorbed by the body over months.

Causes

  • Family history — strong genetic component
  • Prolonged standing or sitting (occupational)
  • Pregnancy — increased blood volume and hormonal changes
  • Obesity
  • Ageing — vein walls and valves weaken
  • Female gender — oestrogen affects vein walls

Symptoms

  • Bulging, twisted blue or purple veins on the legs
  • Aching, throbbing, or heavy sensation in the legs
  • Itching around the veins
  • Swelling of the ankles and feet
  • Night cramps or restless legs
  • Skin discolouration (hyperpigmentation) around the ankles
  • Venous ulcers in advanced cases

Diagnosis

  • Colour Doppler ultrasound — maps blood flow direction, identifies incompetent valves, and plans laser treatment
  • Physical examination — assessing vein visibility and any skin changes

Treatment Options

Endovenous Laser Ablation (EVLA)

Laser fibre seals the faulty vein from inside. Walk-in, walk-out under local anaesthesia.

Radiofrequency Ablation (RFA)

Similar to EVLA but uses radiofrequency energy instead of laser.

Sclerotherapy

A chemical injection scars and closes small varicose and spider veins.

Compression stockings

Non-surgical management to reduce symptoms; does not eliminate veins.

Frequently Asked Questions

Is varicose vein treatment cosmetic or medical?

Both. Even if asymptomatic, varicose veins indicate underlying venous insufficiency that can progress. When symptomatic, treatment is medically necessary.

Will varicose veins come back after laser treatment?

The treated vein is permanently closed and won't return. New veins may develop over time if the underlying predisposition persists — recurrence is 5-10% at 5 years.
