If you are scheduled for surgery — whether for a hernia, gallbladder, appendix, or gynaecological condition — you may be offered a choice between laparoscopic (keyhole) and traditional open surgery. Understanding the real differences between these approaches helps you have an informed discussion with your surgeon and feel confident about the path you choose.

How the Two Approaches Differ

In traditional open surgery, the surgeon makes a single incision — typically 5-15cm depending on the procedure — to directly access and operate on the organ. The view is direct, and the surgeon works with their hands inside the body cavity. In laparoscopic surgery, the surgeon makes 3-4 tiny incisions (each 5-10mm), inserts a thin tube with a camera (laparoscope), and performs the entire procedure viewing a high-definition monitor while manipulating long, precision instruments through the small ports.

Advantages of Laparoscopic Surgery

  • Less post-operative pain — smaller incisions mean less tissue trauma
  • Shorter hospital stay — most patients go home within 24 hours, often the same day
  • Faster return to work and normal activities — typically 1-2 weeks vs 4-6 weeks
  • Minimal scarring — tiny incisions heal to near-invisible marks
  • Lower risk of wound infection and incisional hernia
  • Less blood loss during surgery
  • Magnified view — the camera provides better visualisation of anatomy

When Open Surgery May Be Necessary

Laparoscopic surgery is not always appropriate. Very large tumours or organs, extensive scar tissue from previous surgeries, certain emergency situations, or patient-specific factors (severe heart or lung disease that cannot tolerate the carbon dioxide used to inflate the abdomen during laparoscopy) may make open surgery the safer choice. Your surgeon will evaluate your specific case and recommend the safest and most effective approach.
